Name Change

View and print the name change form and follow the instructions. The requested change will take place within four weeks of our receiving the completed form. Read questions and answers related to the name change process.

• View the form and print it out.
• Complete all required fields.
• Check that you have obtained both signatures (current and new account holder).
• Enclose a photocopy of the new account holder's identification. We will accept a current driver's license, passport or state ID only.
